The definition of U0005 says it is an add-on code with U0003 or U0004. Can it also be an add-on code for 87635, the CPT equivalent of U0003?
At this time, the Centers for Medicare & Medicaid Services (CMS) has indicated that Healthcare Common Procedure Coding System (HCPCS) code U0003 or U0004 are the only primary codes for use with HCPCS code U0005. Because the description of HCPCS code U0005 states that it should be listed separately in addition to U0003 or U0004, a change to the description would be required in order to allow usage with other codes.
